Call for Nominations for the Breathing for Life Award

The Foundation of the ATS invites you to submit nominations recognizing individuals and/or institutions who have made outstanding philanthropic contributions to the ATS and the Foundation of the ATS.

Criteria
The award is presented to individuals or institutions who:

1. Demonstrate support to the specialties of respiratory, critical care and sleep medicine and/or to the Foundation of the ATS.

2. Raise awareness of the burden of respiratory disease on patients and their families  to the public-at large.

3. Play a philanthropic leadership role as volunteer in helping the Foundation of the ATS or the cause of respiratory, critical care, and sleep medicine.

4. Encourage/inspire philanthropy from others, such as other ATS members, non-ATS members and the public-at-large.

5. Are philanthropic in their own communities.

Eligibility

1. Individuals/couples and institutions who meet the specified criteria.

2. Individuals who are not currently on the boards of the Foundation or the ATS.

3. An individual who has been nominated one year will not automatically be carried over for nomination the following year.

4. A nominator may nominate one person(s) or one institution per year.

Selection Process 

1. Nominations may be made by ATS members and individuals otherwise active within the Foundation, the ATS and its chapters.

2. Each nomination must be documented using the attached form and must be signed by the person making the nomination.

3. The deadline for the submission of nominations to the Foundation is December 1, 2009.

4. The Foundation Board of Trustees will review nominations against award criteria beginning on December 15, 2009.

5. Finalists selected for this award will be informed of the honor by January 31, 2010 at the latest. Awardees will be invited to attend the Foundation research dinner and accept the award on Saturday, May 15, 2010.
